Medication Management

Medications play a critical role in the treatment of various mental health issues. The rightmedication can reduce or eliminate symptoms and significantly improve a patient’s quality oflife. For example, they may help stabilize your mood, encourage sleep, improve your appetite, orprevent hallucinations.

Dr. Sayah is keenly aware that the efficacy of medications vary from patient to patient and hewill take into account various factors, including medical history, dosage, side effects andprogress. If a medication management plan is created for the individual, it includes ongoingmedical monitoring of the patients use and progress in order to evaluate the effectiveness of themedication over time. If a medicine you’re prescribed isn’t effective or is causing unwanted sideeffects, your treatment plan will be adjusted accordingly.

Research has shown that both psychiatric medications and therapy are valid treatments, and inmore severe cases the combination of medications and therapy is more effective than eithertreatment alone. Our goal is to work with our patients to achieve relief of troubling symptoms,and then discuss a long-term treatment plan that may or may not include medications managedcarefully. With medication management services, you can communicate right away if your medication doesn’t seem to be working or you notice troubling side effects. Dr. Sayah may theneither switch your prescription or consider a different treatment route.

Telemedicine

SayMed Psychiatry believes convenience has an impact on mental health, which is why we cater to both in-person and online appointments. Telemedicine is a form of video conferencing allowing our Psychiatrist to provide services to patients outside of the office, for example in the comfort of the patient’s home. This allows you to skip travel time, taking time off from work or school, all while still receiving the care you need.

A study published in the Journal of Clinical Psychiatry comparing in-office-treatment to treatment via telepsychiatry indicated “There were no statistically significant differences between study groups for symptoms, quality of life, and patient satisfaction… there is a strong hypothesis that videoconference-based treatment obtains the same results as face-to-face appointments and that telepsychiatry is a useful alternative.”

For your appointment, you will need to have access to a private place and a computer/tablet/smart phone with a camera and high-speed internet. SayMed Psychiatry will provide you with access to a secure HIPAA compliant videoconferencing software free of charge. All you have to do is be on your device and logged on at the time of your appointment.

Ketamine Treatment

Ketamine Therapy at SayMed

Ketamine has been around for over 50 years, and is used in a wide variety of medical settings including as an anesthetic for pediatric and adult anesthesia. It is now being used for people suffering from conditions such as depression, PTSD, bipolar disorder, anxiety, addiction and chronic pain who have tried other medications with little or no relief. Ketamine can produce a wide range of changes to conscious experience, including sensory changes and emotional changes. Used in low doses, ketamine alters levels of glutamate (a chemical messenger) in the brain, which can result in new neural connections. The brain becomes more adaptable and creates new pathways, giving patients the opportunity to develop more positive thoughts and feelings. It has been most widely studied in people with treatment-resistant depression and acute suicidality.

Numerous studies show that ketamine can work as a fast-acting antidepressant, alleviating symptoms in many patients in a matter of hours or days. Many scientific studies have shown ketamine therapy significantly and rapidly reduced depression symptoms in about 50 to 70 percent of patients whereas traditional antidepressants can take four to six weeks to take effect, and for some patients, they don’t work at all.

Ketamine treatment sessions can be administered by an IV, an injection or through a nasal spray. Additionally, we offer virtual reality Ketamine treatments to help build a more calming environment during the treatment. Ways we administer Ketamine

  • Infusion – An intravenous (IV) drip providing medication directly into the bloodstream over a period of 30-45 minutes.
  • Intramuscular (IM) Injection – A shot administered with a needle into the muscle, typically shoulder.
  • Intranasal – A nasal spray administered into the nostrils and absorbed through the nasal sinuses.

Infusion
Our ketamine infusions and treatments at SayMed are customized to each patient. For most patients, we recommend five IV treatments over a two to three week period to get the best results. In addition, should the need arise such as in patients with PTSD and/or anxiety, a small amount of a relaxant can be added to ensure a pleasant experience.

Intranasal
Millions of Americans are estimated to live with depression, of them, one of three do not respondto available treatments. Spravato, is a nasal spray considered to be the biggest breakthrough drugin the battle against depression. The FDA has approved the use of Spravato, a relative of the intravenous anesthetic, Ketamine, originally approved by the FDA in the 1970’s. This medical breakthrough has the potential to deliver immediate results. At SayMed, we treat depression withSpravato, which many consider a form of powerful mediation. With a number of antidepressants we are used to, like Zoloft and Prozac, it takes 4 to 6 weeks to work. Spravato works within the first week. The better a patient gets, the less often administration needs to be done. See if you qualify for insurance.

Does Insurance Cover Treatment?
Spravato is almost always covered by insurance, including Medicare, because it is FDA approved and well-studied to be effective and safe. To be considered eligible for insurance coverage for Spravato, a person has to have tried at least two other antidepressants without benefit.

How is Spravato Administered?
Spravato is administered as a nasal spray and is inhaled through the nostrils. Patients can administer the nasal spray themselves but must be done under the supervision of a healthcare provider.

Virtual Reality Assisted Ketamine Therapy
A patient will be able to see tranquil environments specifically tailored to their likes to create a calming atmosphere. Others may need more guidance and will have a doctor walking them through guiding them while under the VR.
